The European Health Insurance Card in Spain gives you access to medically necessary, state provided healthcare during a temporary stay, under the same conditions and cost as people insured in Spain.
It is often confused with the Spanish health card, but they serve different purposes. The European Health Insurance Card (EHIC) is intended for travel or temporary stays, while a regional health card is associated with living in Spain with recognized healthcare rights.
What does the European Health Insurance Card cover in Spain?
It covers care that shouldn't wait until your return, always within the public system. In practice, this means being able to go to a public center and receive care under conditions equivalent to those of a person insured in Spain, including any applicable co-payments.
To avoid surprises, the key is that the provider is public and accepts the card. If you choose a private center, even if it's near your hotel, that treatment is not covered by the EHIC.
What the European Health Insurance Card does not cover
It does not cover planned treatments, nor does it replace travel insurance. It also does not cover private assistance, and it often excludes what many policies do include, such as repatriation or other typical travel incidents.
European Health Insurance Card and Spanish Health Card: What are the differences?
This table summarizes the most common confusion when someone arrives in Spain or travels through Europe with different health documents.
| Document | What is it for? |
|---|---|
| TSE or EHIC | Provides proof of public coverage for temporary stays in participating countries |
| Spanish health card | Identify the person within the public system in Spain with a recognized right |
If your plan is to live in Spain, the practical reference is usually your health card within the public system, not the EHIC.
How to use the European Health Insurance Card in Spain
Use it as accreditation, not as a pass to any clinic. The European Commission recommends checking that the center is part of the public system and accepts the card before starting the process, which is especially useful in tourist areas with many private options.
If you are traveling from Spain, the application can be made through official Social Security channels, and there is a provisional substitute certificate when it is necessary to prove the right immediately.
