When burning when urinating and urgent urination interrupt your day, the goal is one: fast relief and effective treatment. In Spain, one of the most commonly used drugs for uncomplicated acute cystitis is Monurol (fosfomycin trometamol) in a single 3-g dose, prescribed by a doctor and available in pharmacies. Its short duration of action and urinary activity have made it a practical option for many women.

How does Monurol work and why is it a single dose?

Because it achieves bactericidal concentrations in urine and maintains them for 24 hours after administration, blocking bacterial cell wall synthesis and eliminating common pathogens such as E. coli. Due to its pharmacokinetics, a single 3 g dose is sufficient for uncomplicated acute cystitis.

It should be taken on an empty stomach (2โ€“3 hours before or after eating), preferably before going to bed and after urinating, since food delays its absorption.

In how many hours will you start to feel better?

In pharmacological terms, Monurol begins to act in urine within a few hours and maintains active levels for at least 24 hours; this is why many patients report improvement within the first day, with progressive relief over 24โ€“48 hours. Even so, it is normal for symptoms to take 2โ€“3 days to disappear completely; it is not a sign of failure if the burning sensation gradually decreases. Take it as directed in the package insert: dissolve the sachet in water and drink it immediately, on an empty stomach.

What to do if symptoms do not improve within 48 hours?

Consult again: Do not repeat or change the antibiotic on your own. Your doctor may order a urine culture and assess for causes of nonresponse (e.g., Staphylococcus saprophyticus, which shows intrinsic resistance to fosfomycin, or uncomplicated cystitis).

In some clinical protocols, the specialist may prescribe a second dose or an alternative antibiotic depending on the culture results and your medical history. Seek medical attention sooner if you have fever, lower back pain, visible blood in the urine, pregnancy, or comorbidities.

Monurol provides rapid relief from uncomplicated acute cystitis: a single dose, on an empty stomach, shows improvement within the first day, with symptom resolution within 2โ€“3 days in most cases. If you don't notice a clear improvement after 48 hours, contact your doctor to adjust your treatment based on official sources such as the AEMPS (Spanish Medical Association) data sheet and leaflet.
