In England, an NHS prescription currently costs ยฃ9.90 for each chargeable item, and that amount does not depend on the retail price of the medicine. If the same prescription includes 3 chargeable items, 3 charges apply.
The charge for an NHS prescription does not depend on the price of the medicine.
The NHS clarifies that the charge is per item, not per prescription sheet. Therefore, the useful question is not how much the drug costs at the pharmacy, but how many items subject to reimbursement appear on the prescription.
Not all products are charged the same. Some items are always free, such as contraceptives and medications prescribed to hospitalized patients. This helps explain why the final amount doesn't reflect the retail price of the package.
| Concept | Amount | Application |
|---|---|---|
| Item to be charged | ยฃ9.90 | Per article |

| PPC 12 months | ยฃ114.50 | All recipes |
When does a PPC (Personal Care Plan) make sense for an NHS prescription?
If you pay frequently, the Prescription Prepayment Certificate covers all NHS prescriptions for the duration of your contract. The current price is ยฃ32.05 for 3 months and ยฃ114.50 for 12 months.
According to the NHSBSA, you start saving money when you need more than 3 items in 3 months or more than 11 in 12 months. For someone on repeat medication, it's best to count items that require payment, not packaging.
Who can afford an NHS prescription without paying?
There are groups with free prescriptions, including those under 16, people aged 16 to 18 who are still in full-time education, and those over 60. There are also clinical exemptions and low-income assistance.
Before claiming an exemption, it's advisable to double-check it. The NHSBSA warns that claiming free prescriptions when you're not entitled to them can result in a penalty of up to ยฃ100, even if the error was unintentional.
What happens if you run out of medication?
If you've run out of your usual treatment, the 111 online service allows you to request a limited emergency supply of a medication you take on a repeat prescription. If you normally pay, your usual prescription fee will apply.
This guide refers to England. The NHSBSA notes that NHS prescriptions are free in Scotland and Wales. If you come from Spain and are used to the public health card, it is worth noting that the system works differently in England.
